Patients are welcome to schedule themselves, however please ensure we receive the physician referral before the patient calls. Referrals can be faxed to 206-292-6371.

Required information
Please have the following patient information available when referring a patient:

• Date of birth
• Social security number
• Exam type
• Referring physician
• Insurance carrier
• Diagnosis
• Patient phone number

Notes for referring offices:

• It is important for us to know if the patient has had any previous studies performed in the area being scanned. If there have been previous studies, please fax the reports to 206-292-6371. We will call the imaging site to request the films.
• Please review the exam preparation with the patient, or have the patient call us to get the information.
• We will call the patient the day before the exam.
• While we will be happy to assist in any way with the insurance process, it is still the patient’s responsibility to know what exams their insurance will cover and where their insurance allows them to have the exam performed.
